Medical Clinic - Temporarily Closing

The MEDICAL portion of the MCF Clinic will be closing TEMPORARILY effective 7/1/16.  Physical Therapy and Kris Stoddard are still seeing employees.  We are working hard to get the medical portion reopened as soon as possible.  We will keep you informed.

Rodeo

We will be going to the Spanish Fork Fiesta Days Rodeo on July 21st at the Spanish Fork Fairgrounds.  Please get a ticket sign up sheet from your supervisor.  The deadline for sign up sheets is June 20th.  If there are significant tickets remaining, we will make tickets available to temporary employees, but at this time, MCF employees only.

Mandatory Company Meeting

There will be 2 company meetings for the 1st quarter for ALL MCF Employees.
Thursday, May 5, 2016 for Weekend Employees
Friday, May 6, 2016 for Weekday Employees
3:30 pm @ Spanish Fork High School in the Little Theater (right next to the main auditorium that we are usually in)
Employees only and no food or drink please.  Door closes at 3:30 pm

Tax Form 1095C

To all employees covered by our health insurance for any part of 2015: Form 1095C is required for your income tax preparation for 2015.  These are now available in the Personnel Office.  Forms must be picked up by Thursday 3/31/16.

Payroll

Payroll will likely not be deposited in your account until Monday.  We apologize for the delay.

MCF Clinic Reminders

Employees and their dependents must be responsible to be on time for all clinic appointments (medical, physical therapy, counseling, dietitian).  Persons chronically late or not showing up for an appointment will lose their clinic privileges.

Walk-in patients are welcome, but scheduled patients will be given first priority.
