COVID Swab Test

MCF asked me to go get a COVID swab test done.  What do I do now?

Drive to Springville InstaCare. Follow the signs for COVID testing.  Park in a parking spot.  Call the phone number on the sign.  They will give you directions from here.

After you have done the swab test, download the MyHeath+ app and create a log in.  Check back frequently for your test results.  Do not wait for a call to get your results.  Notify HR 801-894-1118 as soon as you receive your test results.

If you get swabbed on your own, HR 801-894-1118 must be notified that you are being tested.
